When you visit Family Medical Associates of Wyomissing, we hope to establish a mutually satisfying and strong foundation for our relationship with you.
During the first visit, we make sure to obtain important background information, like your medical history, and give you the opportunity to get to know us and our practice.
We are including our basic “housekeeping” rules for a smooth interaction, keeping in mind that we treat our patients as we want to be treated – with dignity & respect for each other’s needs and expectations.
We’ve included a map and directions to our office, practice hours, payment
policies and more.

You can save some time at your first
visit if you would like to print and complete our new patient forms. If you have the ability to scan and edit online, give us a call and we’ll let you know where to
send the documents confidentially.
We ask that you not share private information on our website in order to

What to expect
Being well-prepared for your appointment will ensure that
the doctor has all the necessary information to provide the best possible care for you. Write down your symptoms and take note of the frequency or intensity of them.
Bring a list of questions with you to your visit, so that all
of your concerns can be discussed.
We ask all new patients to bring their list of current medications.
We are also recording Covid-19 Vaccinations.
Please advise our staff of your vaccination dates and specifics.
